Important Information Regarding the Closure of ORTUS MD PA

This is to inform you that ORTUS MD PA will be closing on October 6, 2025.

We understand this may cause some inconvenience, and we want to assure you that we are committed to ensuring a smooth transition for all our patients.

The last day patients are seen at this practice is October 5, 2025.

Your medical records are confidential, and a copy can be transferred to a new physician or provided to you with your authorization. You can access your medical records by completing an authorization form and returning it to PO Box 271827, Flower Mound, Texas 75027.

We encourage you to find a new physician as soon as possible. You can contact your insurance company, the Texas Medical Association, or the local county medical society for recommendations.

After the practice closes, your medical records will be securely stored at Veritrust. A copy of the release form is available at www.veritrust.net. The completed form can be submitted via:
